o Module is On or Off
o CheckBox Status (checked, not checked, always add)
o Data Source (Billing or Shipping)
o Encoding (None, Attr or Entity)
o Email type (Text or Html)
o MailerMailer URL
o MailerMailer key
o Signup Text
o User Attribute #1 through #7 (fixed answers to questions)
o User Comment #1 thru #3 (fixed answers to text questions)
